Director of Social Services Position Overview:

As the Director of Social Services, you will oversee the social work staff in assisting residents with social, emotional, and related fields associated with the medical condition and long-term care goals.

  Director of Social Services Key Duties:

• Ensure the resident(s) have a smooth transition into the facility.
• Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (physical, cognitive, and socialization factors) in planning social services programs.
• Knowledge, skills, and techniques necessary to plan social services programs with the following needs: therapeutic, neurological and dementia, dialysis, IV therapy, and infectious diseases if applicable.
• Maintain progress notes under federal and state guidelines for Long Term Care.
• Promptly follows-up on admission inquires, evaluates, and assesses the needs of prospective residents to determine appropriateness of facility placement. 

Director of Social Services Qualifications:

• Bachelor’s degree required.
• CSW or LSW required.
• Two plus years’ experience with Seniors. Skilled long-term care preferred.
• Knowledge of Local, State and Federal regulations and the survey process.
