Wellness Program Manager designs and implements wellness and work-life programs and policies to build a positive and appealing company culture that will enhance employee engagement, facilitate recruiting, and support productivity improvements. Partners with insurers, healthcare providers, and vendors to introduce wellbeing awareness with on-site health events, classes, assessments/screenings, and information resources. Being a Wellness Program Manager develops and implements work-life programs to provide employees with flexible work arrangements, financial planning, family/child care resources, transportation, memberships, discounts, and educational opportunities. Collects and analyzes information about employment trends and employee workplace needs by conducting surveys or feedback sessions. Additionally, Wellness Program Manager may manage on-site fitness facilities. Typically requires a bachelor's degree or equivalent. Typically reports to a director. The Wellness Program Manager manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions.     Job Summary: The Community Wellness Program Manager (CWPM) facilitates health education and wellness activities that complement and support medical, dental, behavioral health, and social care services for patients and community members. The position works closely with the Virginia Garcia Memorial Health Clinic (VG) Patient Services and Clinical Operations departments, community partners, and dental, medical, and pharmacy teams to assure access to equitable, culturally, and linguistically appropriate, person-centered services. The CWPM plans, develops, implements, and evaluates health education programs and activities designed to support a holistic approach to health and well-being. The CWPM is responsible for the oversight and coordination of the Community Wellness Program to assure an effective model of program delivery that reaches populations with identified needs across the VG service areas.

     

    Essential Duties and Responsibilities:

    Program Management

    · Under the direction of senior leadership, develop a program that delivers community wellness and health education programming that aligns with VGMHC’s mission and strategic goals.

    · Participate in conducting and reviewing community needs assessment and direct the planning and implementation of activities based on the identified needs.

    · Implement VGMHC initiatives and strategic plans for community health education, including monitoring key deliverables and sustainability, in accordance with health center goals and contractual agreements.

    · Work with the Director of Business Services to ensure program models are financially viable and are as self-sustaining as possible.

    · Collaborate with VG operational leadership, Data Team, and Foundation Grants Officers to write and submit reports to federal, state, county, and private funders, including preparing and making presentations on data and program activities to the Board and Executive leadership as requested.

    · Coordinate with Quality team to conduct community and patient experience surveys.

    · Collaborate with Director of Marketing and Communication on planning, marketing, advertising, and product design for community health education program and wellness center events.

    · Conduct outreach to community stakeholders such as instructors, community group leaders, health departments, food banks, universities, school districts, libraries, and others in the community to gain support and cooperation in planning, promoting, implementing, and evaluating wellness and health education programs, and in coordinating their related health education program activities.

    · Maintain contact with public and private agencies, community organizations, and professional groups to enlist community participation and support for wellness activities in the service area.

    · Evaluate the literacy levels of target population and the appropriateness of resource materials.

     

    Operational Oversight

    · Plan and manage staffing of health education and wellness events, services, and activities in consultation with managers and other site leaders.

    · Provide oversight for health education program service providers, volunteers, and staff including scheduling, orientation, observation, performance assessment, and coaching as necessary.

    · Provide coordination and leadership for large team of Navigators and Community Health Workers (in conjunction with their supervisors) across all service lines to assure collaboration, ongoing training, development opportunities, consistent workflows, and updated standard work.

    · Guide workflow development that supports high-quality, person-centered service delivery in environments that may change daily and are fast-paced, flexible, and dynamic.

    · Ensure that all programming and offerings are evidence based and delivered with fidelity to VGMHC approved and legally mandated standards.

    · Coordinate and facilitate meetings and represent the Community Wellness Program through consistent participation in VGMHC leadership meetings and huddles, escalating issues and disseminating relevant information to staff and leaders.

    · Actively engage in VGMHC initiatives, projects and workgroups as assigned.

    · Support mobile programs and services for migrant and seasonal agricultural workers, including helping develop health education for residents of agricultural camps and seasonal nurseries.

    · Investigate complaints and develop appropriate action plans.

    · Oversee and ensure adherence to policies and procedures.

    · Develop and monitor budget and spending for program.

    · Perform other duties as assigned.

    HIPAA Requirements:

    The Program Manager has access to PHI in the course of carrying out their duties. Applying the minimum necessary standard of HIPAA, the designated record sets to which this employee will have access include: all sections of the medical record, patient demographic information in the practice management system, and incoming records, reports, results, consultations, etc. The Program Manager is required to read the content of these records only to the extent needed to accomplish the assigned task (e.g. filing or disclosure).

     

    Knowledge, Skills and Abilities:

    · Committed alignment with VGMHC’s mission, vision, and values.

    · Bilingual English/Spanish preferred (spoken and written).

    · Valid driver’s license, reliable transportation, safe driving record and insurance coverage required.

    · Strong commitment to working with people and staff from a wide range of ethnic, economic, cultural, and social backgrounds.

    · Experience developing, implementing, and coordinating community health education or wellness programs preferred.

    · Experience working in community health, Federally Qualified Health Centers, mobile healthcare, public health, or with special populations including migrant and seasonal agricultural workers, people experiencing homelessness, public housing, refugees, indigenous communities, or other historically marginalized populations preferred.

    · Excellent interpersonal, oral, non-verbal, and written communication skills.

    · Proficient with Microsoft Office applications (Word, Excel, PowerPoint, Outlook).

    · Knowledge of electronic health record system (OCHIN Epic or Epic) is preferred, and competency is expected to be acquired within an appropriate timeframe.

    · Ability to embrace and guide change and innovation.

    · Ability to self-manage and effectively manage workload, including good organizational and time management skills, multi-tasking, and setting priorities in an ever-changing environment while maintaining a calm, professional environment.

    · Ability to work within a team-centered approach and maintain cooperative relationships with multiple stakeholders.

    · Ability to supervise multiple functions and activities and delegate authority and responsibility as appropriate.

    · Ability to establish and maintain effective working relationships with a wide variety of people, including employees, patients, community partners, public officials, and the public.

    · Ability and willingness to work at multiple locations across the broad VGMHC service area.

     

    Education and Experience:

    · Bachelor’s degree in community health education, public health, healthcare administration, or another similar field is preferred.

    · Three years of program management or supervision experience required, preferably coordinating or administering a community health education program, or in health care, public health, or social services program that included program integrity, quality improvement, and systems development.

    · Additional relevant experience may substitute for the education requirement.

    · Experience working in a health care environment strongly preferred

    · Experience with developing and managing new programs valued.

    · Experience supervising and training staff required.

    · Experience managing a program budget, developing staffing plans/models, preparing for external site visits, and reporting program outcomes desired.

    · Experience developing and applying process improvements techniques highly valued.

    · Possible indirect exposure to blood borne Pathogens; expected to observe infection control.

    · Possible exposure to potentially hazardous chemicals.
