The Travel MDS Coordinator will be responsible for utilizing the RAI process to coordinate the completion (including timing and scheduling) of the mandated OBRA and PPS MDS assessments throughout Trio’s Virginia facilities. The Travel MDS Coordinator will use the MDS assessment information to contribute in the development, review, and/or revision of a resident’s care plan as a member of the interdisciplinary team. Serves as a resource for all disciplines in relation to the RAI process. The Travel MDS Coordinator will also function as the key driver for facility reimbursement by ensuring accurate information is captured.
This position will require travel throughout Virginia to provide coverage for 9 Trio SNF’s.
Overnight travel will be necessary at times. Travel expenses including mileage, meals and hotel will be reimbursed per Trio policy. This position works on-site within one of our skilled nursing facility’s in an office environment.
