Transplant Nurse Coordinator provides planning and coordination of all facets of clinical care for potential and actual transplant candidates and recipients. Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Being a Transplant Nurse Coordinator develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Additionally, Transplant Nurse Coordinator requires a bachelor's degree of Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board. The Transplant Nurse Coordinator gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work. Essential Functions:
Coordinates all aspects of the evaluation/listing process for deceased donor and living donor transplantation for Chronic Kidney Disease patients. Coordinates organ procurement with the New Jersey Sharing Network through shared on-call responsibility. Acts as liaison between Saint Barnabas and all referring dialysis centers and referring MDs. Ensures continuity of care through effective communication and collaboration with the multi-disciplinary team in all areas of the transplant department. Uses an understanding of the principles of growth and development to assess each patient s age specific needs and provide age specific treatment. 100% of job responsibilities are pre-transplant no time study is required.
