Transplant Nurse Coordinator jobs in Michigan
Transplant Nurse Coordinator provides planning and coordination of all facets of clinical care for potential and actual transplant candidates and recipients. Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Being a Transplant Nurse Coordinator develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Additionally, Transplant Nurse Coordinator requires a bachelor's degree of Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board. The Transplant Nurse Coordinator gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work. Registered Nurse - Hospice Acute Care Coordinator - Part Time - Days
GENERAL SUMMARY:
Functions as a care coordinator for Hospice within the hospice setting. Assesses and identifies patient needs in accordance with the hospice concept of care. Coordinates with interdisciplinary team to provide complete patient/family care and referral. Provides pain management and symptom control. Maintains the continuity of care by ensuring patient needs are met.

EDUCATION/EXPERIENCE REQUIRED:
- Requires an Associate's Degree or Diploma in Nursing.
- Requires understanding and acceptance of the Hospice concept of care, and a working knowledge of current pain and symptom management.
- Requires interpersonal skills to present a positive image of hospice to those whom are referred to the program.
- Possess the ability to be flexible in adapting policies and procedures to meet the needs presented by hospice patients and families.
- Previous hospice experience preferred.
CERTIFICATIONS/LICENSURES REQUIRED:
- Requires current licensure in the State of Michigan as a Registered Nurse.
- Hospice certification preferred.
