    Description

    This role promotes and supports quality of services within the organization, by completing key administrative tasks related to New Narrative’s Supportive Housing, Clinical, Residential, and Peer operations. The Senior Quality Improvement Coordinator works collaboratively with all staff to ensure high quality data entry, database coordination, dashboard management and utilization management reviews to meet consumer, auditor, contract, and staff requests. The position will focus on the evaluation of data quality to the HMIS and YARDI database systems, consumer satisfaction, the support of Washington County Supportive Housing Services contract compliance, Multnomah County Joint Office of Homelessness funded programs requiring HMIS data entry and coordinate with managers and leadership team members around ad hoc projects as necessary.  

    Essential Duties and Responsibilities 

    Quality Improvement Activities: 

    Program Participant Focus 

    • Coordinates with program participants and assigned SHS staff, in reviewing PHI requests, obtaining necessary documentation, ensuring accurate participant HMIS profiles and service lists, advocacy amongst grievances and consumer feedback, and other projects as necessary. 

    • Assists in consumer survey planning, administration, HMIS data pulls and analysis.  

    • Leads chart reviews for participants enrolled in SHS programs to assess medical necessity, ensures record compliance, data quality and predict training needs. 

    • Supports Health Systems Administrator and Program Analyst in ensuring integration of SHS participant data via HMIS into integrative dashboards.  

    Service Focus   

    • Supports data entry processes for contracted Washington and Multnomah County Supported Housing Services staff through regular utilization management reviews, audits, and coordination with SHS team leads and supervisors. 

    • Responsible for ongoing, in-depth review and evaluation of agency homeless program data collection and practices  

    • Drives continuous improvement of the quality of SHS programs and services through the development, execution, and evaluation of data collection in HMIS.  

    • Supports agency credentialing, including knowledge of tracking clinical credentials for all staff, individual and agency provider enrollment & reporting to payers, maintaining active credentials in EHR, auditing individual staff credential status, and answering staff inquiries regarding credentialing as well as any ad hoc credentialing needs as identified by Human Resources, Billing or Finance. 

    • Assists as needed in investigations regarding critical incidents, grievances/complaints, APS cases, appeals, etc.  

     

    Database Support  

    • Navigates between HMIS and Credible as needed to ensure accessibility of documentation, insurance information, required forms, and billing information. 

    • Data entry for content as needed between HMIS, YARDI and Credible 

    • Initiates and maintains user profiles for HMIS and assist end-users as needed 

    • Provide support and technical assistance for staff around HMIS and Credible systems. 

    • Monitors progress of SHS team members in taking required HMIS trainings 

    • Research, interpret and prepare datasets from HMIS as required. 

    • Develops and generate reports/queries to monitor data quality according to the Washington County Data Quality Plan

    Education/Certificates/Licenses/Registration:  

    Preferred: Qualified Mental Health Associate (QMHA)  

    Experience/Qualifications:  

    2 years minimum of providing administrative support within a healthcare environment. 

    Experience working in HMIS, YARDI or Credible and experience in HIPAA compliance a plus.
