Provides assistance and resources to Community Health Services patients.

Supervision: Support Services Supervisor
Completion of Relias Training and attendance at other educational programs and training is a requirement of employment.
Responsibilities

Maintains strict patient confidentiality at all times.

Provides high quality customer service.

Assess the needs of patients
-
Orients patients to all CHS services and make referrals to the appropriate departments within the organization based on their needs.

Provides information and referrals to patients for social services and community resources.
-
Assists in obtaining language translation services to help patients access medical and community resources.
-
Orients patients to the no show policy.
-
Addresses all no show appeals and bring those appeals to the Performance Improvement committee.
-
Prepares and teaches the no show class.
-
Meets with patients to sign remittance agreements as needed.
-
Maintains required documentation in patient charts as directed by organization policy and procedures.
-
Completes all paperwork and progress reports as assigned in a timely manner and are maintained in a secure location.

Attends agency meetings as assigned.

Participates on organization committees as assigned.

Assists outreach workers by providing organization and community resources for the migrant/season farmworker patients.

Must be knowledgeable of all CHS service guidelines and regulations governing the practice of social work in Ohio.

Attends conferences, workshops, meetings, etc. that promote the organization within the community and provide linkages between agencies.

Facilitates drug rep lunches as assigned.

Assists patients with hardship appeals and responsible for bringing these appeals to the Performance Improvement committees.

Assists patients with completing the sliding fee scale application and collecting relevant income information to support their application.

Assists patients with medication barriers, including patient assistance programs and the 340B program.

Schedules patients for appointments according to protocol.

Confirms appointment on a daily basis.

Performs data entry.

Assists other team members as needed.

Assists in maintaining the organization and cleanliness of the office.

Demonstrates effective communication skills.

Directs patient’s complaints to appropriate personnel.

Responds to crisis situations according to policy.

Maintains knowledge of EHR, practice management system, scanning program, and patient check-in system along with CHS policies and procedures.

Receives and directs visitors and patients.

Attends all required department meetings.

Attends all required staff meetings.

Utilizes supplies in a cost effective manner.

Requests supplies as needed.
-
Adheres to the organization’s standards and policies, including the duty to comply with applicable laws and regulations such as HIPAA, OSHA, OIG guidelines, and other State and Federal laws. This also includes reporting to the Board of Directors, Compliance Officer, Privacy Officer, Administration and/or supervisor any suspected unethical, fraudulent, or unlawful acts or practices.

Qualifications:

Associate’s Degree in social work or related field is required.

Prior experience in working with people of various social and economic backgrounds is preferred.

Ability to function as a leader of a team.

Ability to read and write in English.
