Position Summary: This position is responsible for providing outpatient rehabilitation services in an in-home setting for the older adult population. The in-home setting could be a patient's private residence or in a patient's room in a SNF/Dementia-care facility, in an assisted living facility, or in an independent living facility. This position will evaluate and treat in the most functional environment which is the patient's home and assesses the need for, develop, and deliver speech therapy interventions to facilitate rehabilitation both on an intermittent and maintenance-based scale. The Speech Therapist identifies the need for and makes referral(s) to other disciplines to address the comprehensive needs of patients. The Speech Therapist will implement individualized therapy programs designed to restore, reinforce, and enhance performance and optimize independence. In addition, this position instructs, educates and trains patients and caregivers in those skills and functions essential in promoting quality, independence, and productivity, in order to diminish or correct language and other disorders pathology.

o Therapeutic exercise and interventions for disorders including speech, language, social communication, cognitive communication and swallowing.
o Articulation therapy

o LSVT
o Case Management in the home and community
