Job description
The Patient Care Coordinator is responsible for coordinating care for home care patients and will collaborate with the Clinical Services Manager and Intake Nurse to identify clinicians with the appropriate experience and skill set to match patient needs._ (THIS POSITION IS NOT REMOTE)_
Job Responsibilities:
Assign Evaluation Visits
Process Reschedule Visits, Missed Visits, and Reassigned Visits.
Reassign Declined Visits.
Complete Requested Schedule.
Run Scheduling Requests Report daily, several times a day.
Run Client Schedule Report daily, several times a day.
Run Agent Summary Report.
Review Coordinator notes.
Verify paper visit notes.
Ensure all unscheduled visits have been scheduled at least 7 days out
Complete any additional duties as outlined by the needs of the business.
Ensure compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety, and emergency management
Always maintains patient confidentiality, including all HIPAA regulations
PROFESSIONAL EXPERIENCE/QUALIFICATIONS:
Two years of home care, hospice, scheduling, intake, discharge planning, or case management experience preferred.
Microsoft computer skill set required
Must be computer literate and ability to maintain simple records in English.
Excellent oral, written, and analytical skills.
Should be a skilled organizer able to manage office files and logbooks.
light clerical skills and have a polite telephone manner.
EDUCATION:
· High school diploma or equivalent, Associates's degree preferred
