The RN Care Coordinator position supports the inpatient families at MHealth Masonic Children's Hospital. The RN Care Coordinator role drives timely hospital transitions to home by removing barriers to discharge. Complex pediatric patients may need support from the RNCC with things such as enteral feeds, IV antibiotics, durable medical equipment, arranging home ventilators and trach supplies, extended care nursing, coordination of outpatient specialty care and complex care handoffs to outpatient providers and speciality care coordinators.
The schedule is Tuesday - Friday from 730a-430p with Mondays off. Tuesdays are dedicated to covering inpatinet peds GI/Renal/Oncology teams and Thursdays are dedicated to covering inpatient peds Cards, CV ICU and Heart Transplant. Wednesdays and Fridays will be overflow support or covering absences, etc. After completion of orientation there will be an every 6th weekend of call rotation (9a-3p Sat/Sun) and one holiday off-site call responsibility each year.
AD in Nursing

Past Care Coordination or bedside nursing experience a plus.

Have solid understanding of use of evidence based guidelines.
Demonstrate critical thinking skills, problem-solving abilities, effective communication skills and time management skills.
