Grand Oaks is an Assisted Living Community in Washington DC. Oasis Neighborhood of Grand Oaks is a 24 hour nursing team that specializes in care of Alzheimer's disease and other memory related disorders.

The Nurse Supervisor (Coordinator) leads a team of multi-skilled resident care providers in the delivery of outcome oriented, safe, therapeutic, and cost-effective care. Identifies resident needs based on professional knowledge and medical plan of care. Establishes individualized service plan for each resident’s care. Delegates interventions based on critical thinking skills and a knowledge of the competencies of each team member. Coordinates care to maximize desirable outcomes, resident satisfaction, and cost-effective utilization of available personnel and resources. Serves as a representative of the assisted living and as a clinical role model in all interactions with residents, family, visitors, physicians, and other staff members.

Responsibilities include promoting excellence in resident care in accordance with each resident’s Individualized Service Plan (ISP), programming, providing outreach and support to families, being resourceful and knowledgeable with the Family Resource Library, and the evolving information on Alzheimer’s disease.

Who Should Apply:

1. • Licensure as a Registered Nurse in the District of Columbia.
   • Graduation from an accredited program for Registered Nurses.
   • Minimum of 12 contact hours per year through an accredited provider.
   • Minimum of one-year current experience in an assisted living or long-term care setting.
