The Nurse Supervisor (Coordinator) leads a team of multi-skilled resident care providers in the delivery of outcome oriented, safe, therapeutic, and cost-effective care. Identifies resident needs based on professional knowledge and medical plan of care. Establishes individualized service plan for each resident’s care. Delegates interventions based on critical thinking skills and a knowledge of the competencies of each team member. Coordinates care to maximize desirable outcomes, resident satisfaction, and cost-effective utilization of available personnel and resources. Serves as a representative of the assisted living and as a clinical role model in all interactions with residents, family, visitors, physicians, and other staff members.
Responsibilities include promoting excellence in resident care in accordance with each resident’s Individualized Service Plan (ISP), programming, providing outreach and support to families, being resourceful and knowledgeable with the Family Resource Library, and the evolving information on Alzheimer’s disease.
