    POSITION SUMMARY
    The Outreach and Prevention Education Coordinator will seek to increase stakeholder engagement, inclusive of both organizational partners, internal staff and patients and clients, to optimize health and prevent Human Immunodeficiency Virus (HIV), Hepatitis C Virus (HCV), and Sexually Transmitted Infection (STI) acquisition. This can be done through community education on HIV/HCV/STI prevention activities and staff/client education sessions both internally and externally. Prevention activities include, but are not limited to, the development of a condom distribution program (CDP), Pre-exposure prophylaxis (PrEP) access and education and harm reduction/risk reduction counseling. This position will be responsible for the formation of monthly reports to ensure deliverables are being met and quality improvement initiatives can be identified to improve patient and community outcomes.

    This position reports to the RW Grants Administrator. No other positions report to this position.


    DUTIES & ESSENTIAL JOB FUNCTIONS
    While living and demonstrating our Core Values, the Outreach and Prevention Education Coordinator will: 

    • Provide professional level education via Pennsylvania Certification Board on HIV and Communicable Diseases under Provider #627. Training will be reviewed and updated on an annual basis.
    • Submit required documents for training to the Pennsylvania Certification Board for approval. 
    • Market Pennsylvania Certification Board training throughout the local community and the state of Pennsylvania. Conduct training virtually or in-person, if proper accommodations are available.
    • Provide staff and client information sessions to local organizations serving individuals at-risk on the ID clinic and TWCCH services as well as client information sessions to local organization client base at-risk on the ID clinic and TWC services.
    • Conduct education with outside organizations on HIV & Aging, HIV Stigma, and HIV 101.
    • Conduct community outreach & marketing. This is to also include working with the marketing and communications department to increase awareness of services through social media and marketing materials.
    • Participate in continuing education regularly with equal attendance
    • Represent the ID clinic and TWC at health fairs and conferences.
    • Conduct follow up with all community partners monthly.
    • Keep an ongoing record of all community outreach conducted.
    • Share community resources with case managers at weekly huddles and provide updates via email as they are announced.
    • Organize and promote staff education via use of outside organizations, in-house education programs and workshops.
    • Work with communications to ensure educational material stock is adequate and up-to- date.
    • Work with Supervisor to ensure an adequate supply of promotional items are available for all staff and client sessions, health fairs, and awareness events.
    • Be the lead on outreach awareness events. In addition to organizing other awareness events that coincide with the National HIV Awareness Days calendar or related to Public Health observations.
    • Organize social activities for patients.
    • Responsible for the upkeep of the patient and community resource board that is within the clinic.
    • Responsible for attending and providing information to local Inter-Agency Council (IAC) meetings within each county we serve, where applicable.
    • Participate in the Patient Engagement Committee and actively work to meet the requests of the patient engagement committee, where applicable. 
    • Other assigned office related tasks
    • Perform other duties as assigned

    Requirements

    REQUIRED QUALIFICATIONS

    • Bachelor’s degree preferred
    • Knowledge of HIV prevention education required
    • One year of experience working with people living with HIV, other chronic illness or ethnically, culturally, and sexually diverse population 
    • Experience with MEDENT or other EHR software a plus
    • Proficiency in Word, Excel, PowerPoint and Email- Google
    • Must be reliable and punctual
    • Must be able to perform effectively in a fast paced environment with many changing priorities
    • Must be professional and customer service oriented to deliver quality care and education
    • Ability to work independently and with a team
    • Professional written and verbal communication and interpersonal skills
    • Willingness to work a flexible schedule
    • Ability to support patients with a wide variety of clinical and non-clinical needs
    • Ability to work in a complex, multi-divisional environment with the skills to establish priorities, set objectives and achieve stated goals.
