    Position Summary
    The Community Healthcare Worker (CHW) is “a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the CHW to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. The CHW also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.”
     
    Under the general direction and supervision of the Outreach and Enrollment Supervisor, the CHW will coordinate the care of patients in the community with mental/behavioral illness/crises and acute and chronic medical illnesses, by facilitating access to essential resources, medical services and pharmacologic therapy. Coordination of services is facilitated by frequent two-way communication with the health care team. These services are conducted face-to-face, by phone, outside the clinic, and by telehealth technology as appropriate.

     
    Position Requirements
    Education:  High School Diploma or equivalent
    Certification:  Must successfully complete the Community Health Worker Training within 6 months of hire. 
    Other:  Must possess a valid, state-issued driver’s license
     
    Job Requirements
    ·         Assesses patient needs, health patterns, coping skills, adherence to the designed care plan and makes recommendations to patients’ care team for care coordination needs.
    ·         Educates and coaches patients to help them develop self-care and independence appropriate to their age and developmental level.
    ·         Establishes collaborative partnerships with patients to assist them in examining patterns of health care needs, decisions, lifestyle choices, social determinants of health, and utilization of resources which affect their health.
    ·         Acts as a role model to patients in development of negotiation skills for interactions with health care system and agency personnel that assist them in becoming more active and informed partners in their care choices.
    ·         Collaborates with patient, family, health care providers and community agencies using multidisciplinary, holistic approach to help the patient set goals and achieve outcomes outlined in their care plan(s)
    ·         Works collaboratively with inpatient, outpatient, behavioral/mental health providers, schools, and telehealth settings as appropriate to coordinate seamless care for clients.

     
    Essential Functions, Roles, and Responsibilities
    • Must responsibly, thoroughly and accurately document all visits with patient in Electronic Medical Record (EPIC) within 3 days of patient contact 
    • Complete tasks as assigned in a timely fashion, meeting deadlines when set, take direction from superiors, and remain professional when representing SWMTCHC in the clinic and community
    ·         Responds to patient inquiries and provider referrals regarding information on treatment and support options available in our community in a timely and effective manner

    ·         Demonstrates a thorough understanding of SWMTCHC’s values and mission.

    ·         Provides hope, encouragement, support, and self-determination to patients and assists in their connection to the community

    ·         Assists individuals in accessing services both in the clinic and community that are individualized to meet the patient’s recovery needs

    ·         Advocate for and support patients in a caring and respectful manner while promoting the behavioral and physical health needs of patients
    ·         Other duties as assigned
     
    Required Knowledge, Skills and Abilities
    ·         Effective communication skills.
    ·         Computer skills required on an intermediate level with ability to learn the use of telemedicine equipment and electronic medical record.
    ·         Ability to work independently.
    ·         Excellent decision-making skills required.
    ·         Interest and ability to develop interpersonal communication skills and relationships with others.
    ·         Ability to learn basic patient needs assessment and community-based resources.
    ·         Knowledge of basic personal care, application of adult learning theory, and computer applications.

    Supervision: This position has no supervisory responsibilities
     
    Immediate Supervisor:  Outreach and Enrollment Supervisor; in their absence, Director of Behavioral Health
     
    Physical Demands/Working Conditions:         
    General office/clinic conditions are pleasant; good, clean working conditions where accident and hazards are negligible; requires short periods of moderate lifting, pushing or pulling objects up to twenty pounds. Clear diction and acute hearing are necessary for effective communication with the staff and public. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. 
     
    Position is based in the Dillon and will require travel to patient homes and community partners. Travel to Butte may be required during training. Use of personal vehicle may be necessary for travel; therefore, a valid driver’s license, proof of auto insurance and registration is required.
