    Area of Responsibilities

    ACCOUNTABILITY: Assesses, plans, implements, and directs coordinates patient care to promote and achieve optimal outcomes and improved quality for assigned cohort of patients.

    CLINICIAN AND OUTCOMES MANAGER: Performs comprehensive and ongoing assessment of patient and or caregiver needs in order to plan care to optimize outcomes. Accountable for care delivered or delegated and outcomes of care for designated cohort of patients. Plans coordinates care to improve patient satisfaction, improve staff engagement and satisfaction, decrease readmission rates, improve patient outcomes, decrease errors and near misses and be proactive in avoidance of hospital acquired conditions such as CLABSI, CAUTI, HAPI, injury from falls or other events. Utilizes appropriate quality improvement tools and metrics and evidence-based information to continually improve care and outcomes.

    LEADERSHIP: Develops and implements innovative plans using knowledge, skills, and leadership to create change necessary to improve patient outcomes and quality and decrease costs of healthcare. Serves as a leader in quality improvement initiatives to maximize resources and ensure provision of patient centered, evidence-based care. Participates in the development of new or revision of current nursing policies and practices. Provides leadership in implementation and evaluation of professional standards and policies that impact patient care. Participates in Tanner Professional Practice Model.

    EDUCATOR: Teaches, coaches, and mentors staff related to provision of quality care and improved outcomes. Provides directly or through delegation patient education to promote optimal outcomes. Recognizes need for and actively pursues and shares new knowledge and skills as role and need of the health care system evolve in order to effect change in health practice and outcomes.

    PATIENT AND CARE TEAM ADVOCATE: Adept at ensuring that patients and caregivers are well informed and included in care planning. Creates an environment where professional growth and accountability along with interdisciplinary collaboration can flourish. Partners with Nurse Manger(s) to accomplish common goals. Participates in the Tanner Professional Practice Model.

    Employee performs within the prescribed limits of Tanner Health System's Ethics and Compliance program. Is responsible to detect, observe, and report compliance variances to their immediate supervisor, the Compliance Officer, or the Hotline.

    Required Knowledge & Skills
