Registered Nurse - Home Health
Duties:
- Provide skilled nursing care to patients in their homes
- Conduct comprehensive assessments of patient's health status
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patients' progress and adjust care plans accordingly
- Educate patients and their families on managing their health conditions
- Collaborate with interdisciplinary healthcare team members to coordinate patient care
- Document patient care activities accurately and timely in the EHR system
Requirements:
- Valid Registered Nurse (RN) license in the state of Oklahoma
minimum of 1 year of nursing experience, preferably in home health or a related field
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Proficient in using electronic medical record systems (Axxess experience is a plus)
- Experience in providing care for patients of all ages in their homes and assisted living
- Familiarity with coding and billing processes for insurance reimbursement
