Team Leader - Home Care manages a multidisciplinary team of nurses, therapists, and aides that provide professional services to home care patients. Coordinates clinician schedules and assigns clinicians to patients based on the frequency and expected duration of prescribed treatments and therapies. Being a Team Leader - Home Care monitors utilization, productivity, and tracks patient outcomes. Ensures compliance with agency policies and procedures. Additionally, Team Leader - Home Care typically requires a bachelor's degree. May require an RN license. Typically reports to a director. The Team Leader - Home Care manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. To be a Team Leader - Home Care typically requires 5 years experience in the related area as an individual contributor. 1 - 3 years supervisory experience may be required. We have an immediate need for a full-time Outreach Specialist to join our Care Team.
An Outreach Specialist is responsible for effectively communicating and providing follow up with/to patients, caregivers, physicians/medical providers, pharmacies, medical equipment companies, community resources, and collaborating with team members.
An Outreach Specialist is responsible for coordinating staffing for patients on their team and updating corresponding trackers and platforms.
The Outreach Specialist collaborates with their team and is in the filed with patients as needed.
