Staff Nurse - RN - Psychiatric Unit - Home Care provides professional nursing care to assigned psychiatric patients in a home care environment. Conducts patient intake assessments, develops mental health care plans in conjunction with psychiatrist, and administers medications. Being a Staff Nurse - RN - Psychiatric Unit - Home Care develops therapeutic relationships with patients and records observations of behavioral changes in patient medical records. Helps family members to understand the nature of the condition, and teaches them how to maintain a healthy environment for the patient. Additionally, Staff Nurse - RN - Psychiatric Unit - Home Care typically requires a bachelor's degree in nursing. Typically reports to a manager or head of a unit/department.     Summary:

    Provides nursing care (including psychiatric nursing care) assigned by and under the direction of the certifying physician.

    Essential Duties and Responsibilities: (Note: Other Duties May Be Assigned)

    • Shall be the case manager in all cases involving nursing and therapy care. In cases receiving only physical, speech, or occupational therapy services, or in cases other than those receiving only one or more of the above therapies and home health aide services, that licensed therapy professional shall serve as a case manager.
    • Be responsible for the clinical record of each patient receiving care under his/her responsibility.
    • Assure that progress reports are made to the physician for patients under medical care when the patient’s condition changes and are derivations from the plan of treatment.
    • May assign selected (non-psychiatric) portions of patient care to licensed practical nurses but always retains full responsibility for the care given and for making supervisory visits as required by regulations.
    • Develops home health aide care plan and assigns duties to home health aide but always retains full responsibility for the care given and for making supervisory visits as required by regulations.
    • Responsible for following physician’s orders and reporting changes in patient’s condition to physician.
    • Makes scheduled visits and regularly re-evaluates the patient’s nursing needs, following the plan of treatment and revision when necessary.
    • Teaches, supervises, and consults with the patient and family members in methods of meeting the nursing care needs and other related problems.
    • Initiates emergency measures in life-threatening or unsafe conditions.
    • Participates in all required case conferences, utilization reviews, orientations, in-services, and continuing education meetings.
    • Development and evaluation of the plan of care in partnership with the patient; representative (if any), and caregiver(s).
    • Providing services that are ordered by the physicians as indicated in the plan of care.
    • Preparing clinical notes.
    • Participation in the HHA’s QAPI program.
    • Compliance with the Agency’s Code of Conduct.
    • Patient, caregiver and family counseling.
    • Patient and caregiver education.
    • Communication with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.

    Qualifications:

    • Graduate of an Accredited School of Nursing

    Education and/or Experience:

    The RN must have at least one of the following credentials:

    • a Master’s degree with a specialty in psychiatric or mental health nursing. The RN must have nursing experience (recommended within the last three years) in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program or other outpatient psychiatric services.
    • a Bachelor’s degree in nursing. The RN must have one year of recent nursing experience (recommended within the last three years) in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program or other outpatient psychiatric services.
    • a Diploma or Associate degree in nursing. The RN must have two years of recent nursing experience (recommended within the last three years) in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program or other outpatient psychiatric services.
    • One year of medical/surgical nursing care experience is required. Experience as a public health nurse or home health care experience is desirable.
