Staff Nurse - RN - Pediatrics - Home Care provides professional nursing care to assigned pediatric patients in a home care environment. Records patient histories, performs various diagnostic tests, administers medications, and develops patient home care plans in conjunction with other medical professionals. Being a Staff Nurse - RN - Pediatrics - Home Care performs examinations and prescribed procedures, operating and monitoring medical equipment as necessary. Observes patients' progress and records observations in patient medical records. Additionally, Staff Nurse - RN - Pediatrics - Home Care promotes patients' independence by teaching patients and families to understand conditions, medications, and self-care skills. Typically requires a bachelor's degree in nursing. Typically reports to a manager or head of a unit/department. Requires a valid state RN license. · Provide, facilitate, advocate and promote the best possible care for clients in home setting, assisted living facilities, transitional care units or hospitals
· Seek assistance from physician, NP, PT/OT and/or social services when needed in a timely manner
· Establish a relationship with the client honoring the client’s values and preferences
· Regular contact with personal caregiver to get updated status
· Perform head to toe assessment regularly and follow up with caregiver and clients to determine any changes; Communicate with physician and document changes in client’s condition utilizing both objective and subjective nursing assessment
· Assess risks and needs of clients and identify the need for personal care manager intervention and / or prevention
· Maintain well written client records, progress notes, which support ongoing assessment, and monitoring; Prepare care plans.
· Provide comfortable atmosphere to clients with physical and mental problems
· Supervise functions of home health aide/CNAs/RNs/LPNs and provide education to facilitate best quality of care
· Maintain clients, CNAs, HHAs and Nurses records for Administration point of interest
· Assist Administrator with company’s license renewal and verify employees’ licenses with department of health
· Assist Administrator and community relation personnel with marketing and networking, creating educational brochures for clients and employees
· Assist administrator in keeping track of time records of employees
· Ensure that budget targets are met for recruitment and advertising
· Act as conduit between care givers and administration
· Make sure that client has all possible access to all possible resources in community for well being
· Assist to get company accredited with Joint commission and CAHC
