Staff Nurse - RN - Pediatrics - Home Care provides professional nursing care to assigned pediatric patients in a home care environment. Records patient histories, performs various diagnostic tests, administers medications, and develops patient home care plans in conjunction with other medical professionals. Being a Staff Nurse - RN - Pediatrics - Home Care performs examinations and prescribed procedures, operating and monitoring medical equipment as necessary. Observes patients' progress and records observations in patient medical records. Additionally, Staff Nurse - RN - Pediatrics - Home Care promotes patients' independence by teaching patients and families to understand conditions, medications, and self-care skills. Typically requires a bachelor's degree in nursing. Typically reports to a manager or head of a unit/department. Requires a valid state RN license.     We are looking for a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) in Gallina, NM to administer patient care in accordance with a physician established care plan for 2 patients for 7 hours in total a week.

    This role is a 1099 contract opportunity, with care being provided at the patient’s residence.

    Once our patients are accepted into the program, there is no expiration for this benefit, which allows our team of caregivers to build lasting relationships with their patients for years to come.

    You’ll get to advocate for the patients and work one-on-one to provide them with the care that they need.
    This role will collaborate with CNA’s, HHA’s, Case Managers, and the patient’s physicians.

    Responsibilities:
    • Execute physician prescribed plans of care.
    • Complete compliant documentation of the care provided in the system of record.
    • Manage and administer prescribed medication, treatment, and therapies.
    • Conduct patient assessments, coordination of care.
    • Perform various activities associated with daily living.
    • Monitor vitals and GI intake and output.
    • Assess skin integrity and administer wound care.
    • Monitor for changes in the patient’s condition such as weight loss/gain, self-care abilities, and indicators of disease progression.
    • Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients.
    • Provide education and training to the patient, their family, and/or the Home Health Aide.
    • Work with patients to be done one-on-one in the patients home to provide for a comfortable environment.
    • Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants.
    • Perform other personal care services as necessary to meet the patient’s needs.

    Minimum Qualifactions:
    • Education, licensing, and certification: graduate of an accredited college or university, Associate or Baccalaureate School of Nursing
    • Current State or Compact License as a Registered Nurse or Licensed Practical Nurse.
    • Current CPR certification.
    • TB test and physical examination.
    • Maintain required insurance.
    • Good interpersonal skills.
    • Hand-eye coordination and manual dexterity.
    • Ability to stand or walk for prolonged periods, with the ability to lift up to 50 lbs and move patients.
    • Ability to operate under stressful conditions and make quick decisions.
    • Must have dependable transportation and be willing to commute to the patient’s home.
    • Ability to provide basic medical management up to medically fragile end of life care.

    Preferred Qualifications:
    • 1 years of experience as an RN/LPN in a clinical or home health setting.
    • Ability to assist with two person assist.

    Schedule will be 7 hours a week between two patients. The schedule will be worked out between the patient and the nurse, so the schedule is flexible.
