Looking for an experienced RN to join a home health / hospice team in Oregon

The nursing home is licensed for 40 beds and the affiliated local hospital has 25 beds. Their ER is open 24/7 and they have state-of-the-art facilities at both locations

• Varied Shift Schedule requires weekend, days, nights and on-call

• The ideal candidate will have their BSN (ASN accepted)
• BLS and ACLS certifications both strongly preferred
• Responsible for planning, coordinating and documenting care for an assigned caseload of patients
• Develops and implements a home health plan that meets each patient's specific needs
