The Home Health registered nurse (RN) provides compassionate, evidence-based, and efficient care to individuals in their home.   You will be assigned wherever most needed, as well as based on your experience and abilities.  The home health RN will be traveling to a patient’s home to administer their services as indicated in the physician’s order and/or Plan of Care, collaborate with the patient and their families to attain their goals as indicated in the plan that is structured to help them maintain their independence and meet their goals.  You may be completing an initial or reevaluation of patient assessment & needs to create & modify care plans.   The RN will be providing physical and psychological support to patients, friends, and families.  The RN should be following established professional standards of care, agency guidelines, policy and procedures, and instruct on safety.  The RN’s care delivery is consistent with the Oklahoma Nurse Practice Act, the American Nurses Association (ANA) Scope and Standards of Practice, and the ANA Code of Ethics.  

Responsibilities:

• Promotes patient’s health by completing the nursing process including assessment, analysis of data, establishing goals, prioritizing and implementing interventions, and evaluating outcomes
• Provide services such as administering at-home IVs, changing dressings, cleaning wounds, as well as education and training the patient and families on proper home health care procedures and strategies
• Communicates, updates, and collaborates with physicians and the multidisciplinary home health team involved in the care
• Identifies patient care needs by establishing personal rapport with patients and their families and provides emotional, psychological, and spiritual support
• Identifies patient and family education needs and utilizes appropriate resources to meet those needs
• Assures quality of care by adhering to therapeutic and regulatory standards; administers medications and treatments prescribed by any person authorized by state law to prescribe
• Protects patients and team members by adhering to infection-control policies and protocols; medication administration and storage procedures; and controlled substance regulations.
• Documents patient care services timely and accurately
• Maintains patient confidence and protects operations by keeping information confidential.
• Delegating tasks as may safely be performed by others, consistent with educational preparation and that do not conflict with the provisions of the Oklahoma Nursing Practice Act
• Ensures operation of equipment by verifying preventive maintenance requirements are met; calling for repairs; and evaluating new equipment and techniques
• Maintains professional and technical knowledge through continuing education opportunities including internal and external educational offerings
• Maintains a cooperative relationship among health care teams by communicating information and participating in team problem-solving methods
• Travel in a company or own vehicle to multiple homes and facilities will be necessary to provide the care and services to the patients
• Apply DRH Standards of Behavior and exhibit behaviors consistent with DRH core values
• Performs other related duties as assigned
