Staff Nurse - RN - Home Care Admissions evaluates patients referred to home care services and determines the types of services to be provided. Interviews the patient and family to determine appropriateness for services. Being a Staff Nurse - RN - Home Care Admissions obtains initial and ongoing referral documentation from physicians, insurers, and providers. Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Staff Nurse - RN - Home Care Admissions initiates care with service providers. Completes admission, referral, and insurance forms. Utilizes health record management software. Typically requires a bachelor's degree in nursing or equivalent. Requires an RN license. Typically reports to a manager or head of a unit/department. Staff Nurse - RN - Home Care Admissions's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. Client is a young adult who lives near the mouth of Parley's Canyon. Cares include pain meds and TPN feedings.
Available Shifts: Monday - Sunday, 6 hours in the evening (flexible start and end time)

Providing thoughtful care to client's based on personalized Plan of Care
Providing in-home assessments for client's
Monitoring and tracking changes in the client's conditions and needs, and keeping the Director of Nursing and any other appropriate personnel informed
Proactively updating the client’s family on plan of care, health changes, and concerns
TPN Feedings and additional tasks as needed
What Do You Need to Be Successful in This Role?
Certified CPR and negative TB skin test or chest x-ray
Have a minimum one (1) year of documented experience in a home health agency, primary care clinic, or health facility, preferred.

Be an awesome driver with your own reliable vehicle and a valid drivers license
