Staff Nurse - RN - Home Care Admissions evaluates patients referred to home care services and determines the types of services to be provided. Interviews the patient and family to determine appropriateness for services. Being a Staff Nurse - RN - Home Care Admissions obtains initial and ongoing referral documentation from physicians, insurers, and providers. Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Staff Nurse - RN - Home Care Admissions initiates care with service providers. Completes admission, referral, and insurance forms. Utilizes health record management software. Typically requires a bachelor's degree in nursing or equivalent. Requires an RN license. Typically reports to a manager or head of a unit/department. Staff Nurse - RN - Home Care Admissions's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. BAYADA Home Health has an exciting opportunity for a Full-Time Admissions Registered Nurse!! You will complete OASIS Starts of Care (SOC) for adult and geriatric patients in Providence, Cranston, Warwick and surrounding towns. One year prior clinical experience as a licensed RN is required and Home Health and OASIS experience is strongly preferred. You may be eligible for a $5,000.00 sign-on bonus!

Registered Nurse (RN) Responsibilities:
- Complete OASIS Starts of Care (SOC) for adult and geriatric patients
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a touch pad tablet
Qualifications for a Registered Nurse (RN):
- A current license as a Registered Nurse in Rhode Island
- Graduation from an accredited and approved nursing program.
