Staff Nurse - RN - Home Care Admissions jobs in Pennsylvania
Staff Nurse - RN - Home Care Admissions evaluates patients referred to home care services and determines the types of services to be provided. Interviews the patient and family to determine appropriateness for services. Being a Staff Nurse - RN - Home Care Admissions obtains initial and ongoing referral documentation from physicians, insurers, and providers. Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Staff Nurse - RN - Home Care Admissions initiates care with service providers. Completes admission, referral, and insurance forms. Utilizes health record management software. Typically requires a bachelor's degree in nursing or equivalent. Requires an RN license. Typically reports to a manager or head of a unit/department. Staff Nurse - RN - Home Care Admissions's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. Available Shifts:
Must be willing to do admissions!
Short and long visits/shifts are also available
We are willing to work with the employee's schedule and can give as little or as many hours as he or she wants. Part time or full time is available.
Responsibilities and Duties:
Provide one-to-one care in the home
Take directions from clinical team
Follow plan of care
Directs and guides patient/family teaching and activities while maintaining standards of care
Communicates with the physician about changes in the patient's clinical condition.
Ability to respond quickly and accurately to changes in condition and/or response to treatment.
Follow all clinical and office policies
Job Requirements:
· RN License
· Clean background and criminal record
· CPR certified
· Knowledge of computerized documentation and office software required
