Staff Nurse - RN - Home Care Admissions jobs in Connecticut
Staff Nurse - RN - Home Care Admissions evaluates patients referred to home care services and determines the types of services to be provided. Interviews the patient and family to determine appropriateness for services. Being a Staff Nurse - RN - Home Care Admissions obtains initial and ongoing referral documentation from physicians, insurers, and providers. Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Staff Nurse - RN - Home Care Admissions initiates care with service providers. Completes admission, referral, and insurance forms. Utilizes health record management software. Typically requires a bachelor's degree in nursing or equivalent. Requires an RN license. Typically reports to a manager or head of a unit/department. Staff Nurse - RN - Home Care Admissions's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. Registered Nurse (RN) Case Managers oversee all medical services our clients receive which includes nursing, physical / occupational therapy, and home health aides. This begins with a comprehensive clinical assessment and creation of a specialized care plan, consistent with recommendations from the hospital, skilled nursing facility, and primary care physician. Direct nursing services include periodic health assessments, teaching, skilled intervention, care coordination, and medication management. All of this takes place in the client's home!
Utopia provides a full spectrum of nursing services such as Medical / Surgical, Wound Care, Orthopedic Care, and Behavioral Health. This position requires patience, compassion, and a desire to care for those who need it most, just like family. If providing care for seniors and clients with disabilities comes from your heart, this position is for you!
We are especially looking for:- Behavioral Health experience
- At least 1 year of experience in a home care setting
- Bilingual in English and Spanish
No home care experience? That's ok! We have an assigned nurse educator who provides training as part of the on-boarding process.
- Sign on Bonus paid $1500 at 6 months and $1500 at one year.
