Salary:

Starting at $58,000.00 (Exempt)

About the Position:

Support the Care Coordination program throughout the agency service area. Provide support, information, and direction to care coordinators. Ensure program philosophy, policies, and procedures are implemented and enforced.

A Typical Day as a Care Coordinator Supervisor:

• Ensure that individualized services and support are provided in accordance with the unique potential and needs of each child and family.
• Evaluate all care coordination services provided to ensure the services are meeting long-term objectives of the agency and meeting standards of time and cost efficiencies.
• Review and approve records, reports, and documentation from supervised staff to ensure accuracy and compliance with guidelines and procedures.
• Provide weekly one-on-one supervision with care coordination team.

Does the Following Apply to You?

• Master's degree in Social Work, Marriage and Family Therapy, Psychology or related field
• Mental Health or Human Service Experience (preferred)
• Supervisor Experience (preferred)

Additional Qualifications:

• Must obtain and maintain IM CANS certification. Must complete and maintain all required training and certification as required by HFS. Must complete training on the provision of supervision.
• Completion of DCFS criminal background check and fingerprinting
• Must be at least 21 years of age
• Obtain an insured vehicle
• At least three years of driving experience with a valid driver's license
