The Ambulatory Care Coordinator is a Registered Nurse (RN) directly responsible for assessing, coordinating and integrating healthcare for patients with chronic conditions in the Aspirus Clinic System. This RN position manages all aspects of patient centric care coordination through collaboration with physicians, staff and other health professionals to provide services across the health care continuum for all patients within the clinic setting. The Ambulatory Care Coordinator will work directly with the entire health care team to deliver innovative population health management practices to the patients and community of Aspirus.
HOURS: Full Time or 1.0 FTE, 80 hours per pay period, day shift
