POSITION TITLE: Advanced Practice Psychiatric Nurse

JOB DESCRIPTION: Provides psychopharmacological consultation to primary care and behavioral health staff and provides psychiatric care to patients in an out-patient primary care setting.

POSITION SUMMARY: APRN works as part of the patient’s primary care treatment team and provides support in managing the psychological aspects of the patient’s care.

REPORTS TO: Behavioral Health Director with shared oversight of medicalpractice by the Chief Medical Officer

FLSA STATUS: Exempt

PRIMARY RESPONSIBILITIES:

• Support each primary care team with psychopharmacological perspective on patients
• Carry a clinical caseload and provide direct clinical services to health center patients (e.g. psychopharmacological consultation, diagnostic evaluation, and medication management)
• Participate in daily staff ‘huddle’ and “warm hand-off”
• Supervise other staff and trainees as needed
• Attend primary care treatment team meetings
• Attend weekly consultation meetings with BH Director
• Participate in staff training around motivational interviewing, behavioral management of complex medical and psychologically impacted patients
• Complete comprehensive documentation for each patient visit in the patient’s health record
• Demonstrate a commitment to integrated health care by at least quarterly attendance at oral health department meetings

REQUIRED SKILLS, EDUCATION & EXPERIENCE:

• Strong commitment to CHCFC Mission. Strong interpersonal and communication skills. Ability to work with a diverse population and sensitivity to low-income populations.
• Registered Nurse licensure in MA
• Advanced Practice Registered Nurse licensure in MA
• Current health psychology licensure in MA
• Five years of related health care experience

WORKING CONDITIONS:
Able to work in a multidisciplinary team environment. While performing the required duties of this job, the employee is exposed to weather conditions prevalent when traveling between Health Center locations where medical care is provided. This position performs tasks that involve exposure to blood, body fluids or tissues.
