BAYADA Home Health Care is currently seeking an energetic and experienced Medical Social Worker for a per diem opportunity performing hospice visits our Norwich, VT Hospice Office. This office services clients on a per visit basis in territories throughout Windsor and Orange, VT and Grafton, NH counties.
Prior home care experience preferred, but not required. As a Medical Social Worker, you will be an integral member of a multi-disciplinary hospice care team that provides skilled nursing, social work and palliative care to clients, affording them the opportunity to receive the medical care required to remain at home.
