Provides crisis intervention and assists families in understanding the implications and complexities of the life situation and its impact on lifestyle. Being a Social Worker (MSW) provides professional social work assistance to clients and families as related to illness, disease, hospitalization, financial and educational needs by utilizing a variety of modalities in all areas of service. Responsibilities also include involvement in the discharge planning process. Additionally, Social Worker (MSW) requires a master's degree. Typically reports to a manager. Overview
Provides assessment, counseling, information and referral, advocacy services, and other resource assistance and support to patients directly or through consultation for the Ambulatory Neurology Clinics.
Responsibilities
- Provides complete psychosocial assessments of patients and families.
- Provides counseling to individuals and families to help solve social and emotional problems.
- Coordinates community resources to address/meet identified needs.
- Assists families in adapting to difficult and complicated dispositions involving complex/catastrophic care needs, multiple agencies/providers and linkage to resources outside the community.
- Provides consultation, assessment, referral and coordination in cases of suspected child or dependent adult abuse or neglect, or other forms of domestic violence.
- Organizes support groups.
- Documents objective, concise and pertinent clinical data in the medical record.
- Perform other duties as assigned.
- Master’s Degree in Social Work (MSW) required.
- Social work experience in an acute, home care, or ambulatory setting preferred.
- Strong organizational skills, verbal and written communications skills.
- Sound decision making and judgment.
- Time management skills.
- Negotiation skills.
- Proven ability to deal with conflict resolution.
- Comfortable with basic computer skills.
