Position Summary
Provides assessment, diagnosis, treatment and crisis intervention as needed to members who present themselves for psychiatric evaluation.
Position Accountabilities
Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
Participates as a member of the multidisciplinary treatment team, more particularly with emergency room patients.
Provides crisis intervention as needed, as well as emotional support, counseling and discharge planning to patients/families with diverse populations.
Prepares and maintains documentation as required on each case.
Provides assessment of social and emotional factors related to the patient’s illness.
Provides assessment of the patient’s home situation, financial resources, and/or provide referral to community resources that are available.
Keeps Director informed of problematic cases, especially those involving legal or risk management issues. Maintains confidentiality of all patient information.
Evaluates and documents needs linked to the patient’s medical condition, physical, mental, emotional and social, requiring the social worker to intervene.
Consults with physician and other members of the health team for the purpose of assisting them to understand significant social, emotional, and environmental factors related to the patient’s health problems and planning and directing each individual member’s treatment program.
Provides rehabilitative and supportive casework and assists the multi-disciplinary team in order to help patients and families cope with a patient’s hospitalization, illness, diagnosis, treatment, and /or life situation, including emotional, mental and substance abuse disorders and geared toward restoring patients to their optimum level of social and health adjustment. This includes assisting patients and their families to understand and accept their health problems, and follow medical recommendations. Work with patients and their families concerning secondary effects of illness, especially social and emotional factors.
Helps patients utilize the resources of their families and the community. Researches alternatives to tertiary care, such as transitional, rehabilitation, long term, mental health, home health care, and community resources. Refers patients to resources such as but not limited to referral for home health, durable medical equipment, Dept. of Health & Human Services (DHS), Hospice, etc.
Makes referrals to case manager, as appropriate, and/or refers member’s family to community support services and resources. Utilizes resources of public and private agencies and community organizations to meet the needs of members.
Applies clinical criteria to patients admitted to ensure accurate, billing compliance for all payers.
Reports all cases to (DHS) involving allegations of sexual or physical abuse, neglect & intimate partner violence.
Maintains clear and timely communication with the healthcare team, including documentation in the medical record to facilitate comprehensive, efficient, and appropriate discharge.
Participates in patient case conferences and continuing evaluation and revision of treatment plans.
Attends meetings as required and participates on committees as directed
Enhances personal growth and development through participation in educational programs, current literature, in-service meetings and workshops.
Performs other related duties as assigned or requested.
