Assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet clients’ psychological, social, environmental, and physical needs. Other responsibilities include evaluation of service efficiency and excellence, provision of social support, crisis support, and client education. Actively participates and facilitates tracking of cost, resource management and quality improvement parameters. Assists in screening, third-party payor authorization, and discharge activities providing linkages to health and wellness resources across the health care continuum. Uses social services expertise and problem solving skills to achieve optimal social, psychological and resource outcomes within effective time frames. Promotes interdisciplinary collaboration with a team approach. Provides discharge planning services. Participates in Risk Management. Makes indicated client referrals. Supports physicians in meeting the social needs of clients.
Bachelor’s degree in Social Work from an accredited program is required. Licensure in Social Work in the state of Kentucky is preferred. Certification in area of expertise preferred. EXPERIENCE: Two years in a hospital setting or three years experience in casework. Must have experience in group technique and discharge planning. Computer Literacy. Excellent Communication skills, written and verbal. Excellent Customer Service and problem-solving skills.
