LIV Health is looking for an experienced, motivated, outgoing individual to provide case management services in Laramie County. The position is full-time and open to a BSW or CSW. Must be willing to travel and work from home with equipment provided.

Please note this position is for LEAD Case Manager for our Cheyenne and Laramie Case Management Teams so travel between the two areas will be required.

Job duties include meeting with patients in their homes or other community settings, completing intakes/assessments/screenings, making referrals, assisting patients with paperwork, accessing community resources and managing patients on the CCW and DDW Medicaid Waiver programs. Must have a knowledge of community resources, such as Medicaid, SSDI, SSI, SNAP, CHA, waiver programs, and medical social work. Must be able to work with interdisciplinary team and community partners. Lead Case manager will be expected to lead staff meetings, phone meetings, community meetings and have good time management skills.

Pay: $54,000.00 - $60,500.00 per year
