PACE@Home is seeking a Social Work Case Manager to provide direct social work case management services to PACE participants and to provide social services support to include participant screening, case management, counseling and referral.
Employee Type: Full-Time
Minimum Experience: Must have two years experience in a health related - medical setting and one year experience working with the frail and elderly population.
Required Degree: Master’s Degree from an accredited School of Social Work. Must maintain CPR and First Aid Certification.
Other Requirements: Must have Medical form filled out and signed by physician, PA or NP prior to working.
