Social Work Manager plans and implements programs to meet the social and emotional needs of patients and patients' families in a health care setting. Develops plans for patient care after release from treatment. Being a Social Work Manager provides social services to patients/clients and their families. The Social Work Care Manager provides inpatient care coordination services for medical/surgical patients to facilitate optimal patient outcomes, supports the delivery of effective and efficient care, improves continuity of care and efficient use of resources throughout the episode of care. The care manager will collaborate with the care team including, but not limited to: attending/specialty physician(s), social work, utilization management, therapies, and nursing to ensure that an effective and timely treatment plan is acted upon, and appropriate discharge plan is implemented with a smooth transition in levels of care. This position requires periodic weekends and/or holiday(s).
Work Schedule: Casual - Work on-call Friday from 4:30-8:00 pm, work Saturday and Sunday from 8:00 am to 2:00 pm every four weeks, and have availability to fill in on weekdays 8:00 am-4:30 pm for vacation and sick calls.
