Social Services Director - Nursing Home jobs in Shawnee, KS

Nursing Home Experience Required. Social Work license preferred.

Excel Healthcare Roe Park is currently recruiting a Social Services Director to join our team! A license in Social Work in the State of Kansas is preferred. The successful candidate must have long term care experience working the the geriatric population to be considered for this position.

Job Description and Responsibilities include but are not limited to:

• The successful candidate will assess, plan, coordinate and implement services to enhance our resident's social and psychosocial well-being and assure that the highest degree of quality care is provided at all times.
• Perform all social and psychosocial functions. In cooperation with the Administrator and DON, evaluate resident's psychosocial and social needs.
• Establish psychosocial goals to be included in resident's plan of care in conjunction with nursing and other departments and provide ongoing assessment of resident's well-being and discharge potential to a less restrictive environment.
• Provided a continuing assessment of resident care plan by direct contact with family members on a regular basis and maintaining a friendly, supportive relationship.
• Notify family/responsible party of scheduled care conferences, encourage them to remain involved in care and life of resident, and inform them of any change in resident's physical well-being.
• Conduct MDS's and assessments in a timely manner as needed and act as the Grievance Committee Director.
• Coordinate ancillary services in conjunction with nursing staff

• 8 hour shift
• Monday to Friday
• MOD (Manger on Duty) one weekend day a month
