POSITION SUMMARY:
The Director of Social Services is responsible for providing related social work services so that each resident may attain the highest practicable level of physical, mental and psychosocial well-being. The person holding this position is delegated the responsibility for carrying out the assigned duties and responsibilities in accordance with current existing federal and state regulations and established company policies and procedures.
ESSENTIAL DUTIES/RESPONSIBILITIES:
Role Responsibilities – Job Knowledge/Duties:
- Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning social services programs.
- Demonstrates knowledge, skills, and techniques necessary to plan social services for residents with the following needs; therapeutic, neuro and dementia, dialysis, IV therapy, and infectious diseases if applicable
- Acts appropriately under the direction of the Administrator and acts as an active member of the interdisciplinary team
- Demonstrates ability to adjust to changes in unit/shift assignments to meet resident and family needs
- Communicates and observes the Corporate Compliance Program effectively and complies with Code of Conduct when performing work functions
- Other duties as assigned by the Administrator
Role Responsibilities – Administrative Duties & Resident Care
- Supervises, trains, evaluates, counsels, and disciplines inter-departmental personnel
- Assists in the recruitment, interviewing, and selection and termination of social services personnel
- Provides orientation to long term care facility and services rendered, to residents and family members
- Arranges inquiries, evaluates and assesses the needs of prospective residents to determine appropriateness of facility placement
- Schedules appointments with residents or responsible party, guardian, next of kin, sponsoring agency or agencies or lawful representative to:
- Review and interpret Resident’s Rights, including facility Policy and Procedure implementation. Obtain signatures, sign as a witness to signatures, and provide client or above-mentioned representative with copies
- Review and explain facility Advance Directives policy. Obtain signature, sign as witness to signature, then provide residents or representative copies of DNR regulations, rights, duties and responsibilities
Demonstrates responsibility for resident transfers to the following:
- To Hospitals:
- Completes inter-facility transfer form prior to transfer. Sends transfer form to hospital to assist them in understanding the psychosocial behavior of the resident
- Checks with responsible party for confirmation of bed reservation if resident has private resources. Informs both Business office and Nursing office of determination of resources
- Maintains communication with the hospital regarding resident progress
- Documents resident admission to hospital an approval for bed retention if resident has own resources
- Notifies all disciplines immediately if a resident is a recipient of Medicaid funding. Notifies hospital and family or responsible party that bed reservation is canceled
- From Hospital:
- Obtains updated information over the telephone from Hospital Discharge Planner to prepare various departments of incoming resident’s needs
- Screens residents at the hospital with the Director of Nursing when there is a question regarding specifics of care to be provided, resident’s readiness to be discharged to nursing home, or level of care needed
- Provides pre-screening with the Nursing Department of all HRF re-admissions to the facility
- Notifies all departments of incoming residents
- Updates any new assessment information on resident’s chart
- Inter-Facility Transfer (Room Change):
- Obtains consent from family or responsible party upon Utilization Review committee decision (other than emergency transfer authorized by physician) for those residents in receipt of Medicaid funding
- Charts decisions reached of lower or higher level of care by the Care Planning committee or by the attending physician in compliance with various departments
- Notifies departments of resident transfers
- Discharge to Hospital:
- Completes Discharge Summary to send to the hospital Social Work Department, as requested
- Assists the hospital with their discharge planning procedure and continuity of care for the resident (except in case of expiration), or communicates with the hospital (via the telephone) to share resident information
- Discharge to the Community:
- Obtains current release of information
- Conducts a discharge planning conference at the discretion of the planner, and assists resident and family members/responsible party in preparation of discharge
- Issues pending discharge bulletin for resident
- Formulates post-discharge summary, and distributes to appropriate departments
- Expiration:
- Completes a discharge summary and sends to the Discharge
- Assists family members or responsible party with any necessary arrangements
Role Responsibilities – Documentation:
- Completes Social History after conference with client, family and/or responsible party within 72 hours upon admission and/or re-admission
- Completes Social Service Review for MDS in accordance with MDS schedule
- Completes the initial psychosocial assessment section of Social History within fourteen days of admission
- Completes the Screening Assessment for Indicators of Aggressive and/or Harmful Behavior within 72 hours
- Completes the Screening Assessment for Evaluating Self-Harm/Suicide Risk within 72 hours
- Completes the Elopement Risk Review within 72 hours
- Completes the Discharge Planning Review within 14 days
- Completes the Community Survival Skills Assessment within 72 hours
- Maintains significant social service progress notes on the resident’s medical chart on a timely basis and, at least quarterly, completes a progressive assessment note
- Maintains a current social service plan and discharge statement on the resident Kardex
- Completes a discharge summary within 48 hours of discharge
- Documents a progress notation regarding discharge/transfer and the reason for relocation, date, time facility to which transferred and method of transportation and readiness intervention
- Records all admissions and discharges in resident registry book
- Maintains separate file on every resident to be filed in the Social Work office
- Active involvement in care planning, discharge plans and resident rights
Role Responsibilities – Personal Needs and Services:
- Counsels residents regarding psychosocial programs
- Works directly with family or responsible party to enhance functioning of resident in the facility
- Assists with financial needs of resident, stressing confidentiality of records, and notifies proper facility departments of Medicaid coverage
EDUCATION/EXPERIENCE:
- Either a Bachelor’s Degree in Psychology or Sociology; a B.A. or M.A. in Social Work; or a Licensed Clinical Social Worker’s Certificate (Preferred)
- Must be capable of performing the essential functions of the job, with or without reasonable accommodations
- Must be able to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required
