Founded in 1976, Tumbleweed is a non-profit, community-based agency serving homeless, trafficked, runaway, and otherwise at-risk youth and their families and support systems.

    Job Summary:

    The John H. Chafee Foster Care Case Manager provides Chafee Independent Living Services to help current and former foster care youths achieve self-sufficiency. Services are offered to meet the individual needs of youth and include CFCCM service elements.? CFCCM services will be provided through life skills groups, individual meetings, or in collaboration with foster parents.

    Core Job Requirements:

    1. High School diploma required; bachelor’s degree in human services or similar field with two years of experience with a variety of populations (mentally ill, substance user etc.) or a combination of education and experience is preferred.

    2. Must have knowledge and basic abilities in interventions that are rooted in Positive Youth Development, Trauma Informed Care and the Youth Thrive guiding premises.

    3. Must possess a valid driver’s license, proof of insurance, and must meet insurability requirements of agency automobile insurance policy.

    4. Knowledge of needs of youth in crisis and/or transition and community resources

    5. Computer skills for data recording and record keeping.

    6. Strong interpersonal skills with the ability to problem solve, be compassionate, firm and to deal calmly in crisis.

    Essential Functions of the Position:

    Meet monthly and as needed with youth to discuss current needs and review transitional living plan (TLP) and revise as needed every 6 months.

    Track all services provided to youth and submit service logs monthly to comply with the National Youth in Transition Database.

    Ensure that all necessary forms and releases are completed specific to your case file.

    Provides case planning/management.

    Advocates and refers to other community resources as necessary.

    Track data related to in person interactions with youth, progress youth is making on their TLP, immediate or pressing emotional/physical health of youth, youth's current living situation, current contact information, effort being made to re-establish and/or maintain connections with family members as well as additional pertinent information.

    Generate monthly progress reports and reports to CPS worker when appropriate.

    Maintain contact with youth's CPS worker at minimum monthly.

    Completes the Casey Life Skills Assessment with each youth at minimum annually.

    Participates in case staffing/in-service training meetings, and professional development activities.

    Completes all job/case related forms and reports based on the appropriate timelines outlined by Tumbleweed protocol and or supervisor, and the maintenance of client records in DAP format.

    Enters data into required databases in a timely manner as defined by Tumbleweed protocol and or supervisor.

    Performs other duties as assigned by supervisor.
