Residential Care Medical Director plans and directs all aspects of clinical care at an assisted living or skilled nursing facility. Ensures that clinical programs operate in compliance with all applicable regulations. Being a Residential Care Medical Director provides patient consultations as needed and manages relationships between staff and attending physicians. Advises non-medical management on clinical and patient related matters and policies. Additionally, Residential Care Medical Director requires a MD degree from an accredited school. Requires a state license to practice medicine. Typically reports to top management. The Residential Care Medical Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Residential Care Medical Director typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function.     Responsibilities

    Position Summary:

     

    The Director of Care Coordination is responsible for the oversight of care coordination and social services, including but not limited to clinical resource management, discharge planning activities, patient advocacy, clinical social work and best practice in medical necessity determination and concurrent review.  The Director will support the Organization's strategic plan and the goals and direction of the Performance Improvement Plan (PIP).

     

    Principal Duties and Accountabilities:

    • Responsible for overall direction and management of care coordination areas including planning, organizing and directing all activities, staffing, performance improvement in the delivery of clinical services (such as LOS reduction) and reporting needs within the Medical Center and Dignity Health, as well as government and regulatory reporting. Participates regularly in Medical Center, division, or corporate meetings pertinent to the accountable areas and participates in strategic planning and performance improvement teams as necessary.
    • Guides care coordination activities according to the needs, requirements, and policies of the Medical Center, the affiliated medical groups and health plans, Dignity Health, any Federal and State agencies, and according to standard practices of the professions under the Director's accountability. Considers the population served by the Medical Center and area clinical integration programs and leads efforts to optimize care coordination across the care continuum and ensures a plan of care for patients in all stages of health needs.
    • Consults and collaborates with other managers, physicians, administration, and community based healthcare workers regarding care management issues identified through corporate or facility initiatives and current literature.
    • Works closely with all departments at the Medical Center and the post-acute service providers to streamline the patient transition through the health care system and into the community post discharge.

    Qualifications

    Minimum Qualifications:

    • Bachelor's degree in nursing or MSW required.
    • Minimum of three (3) years of progressive supervisory or management experience in an acute hospital setting.
    • Current Registered Nurse (RN) license or LCSW license (or equivalent license) in state of hire.

    Preferred Qualifications:

    • CM certification preferred.
