Resident Care Coordinator coordinates, supervises, and evaluates the care of residents in a skilled nursing, assisted living, or similar facility. Schedules and trains care staff, and ensures that clinical services are delivered in accordance with regulations and professional standards. Being a Resident Care Coordinator communicates with families and medical professionals as needed, and ensures care plans for residents are properly documented and updated. Requires a high school diploma. Additionally, Resident Care Coordinator may require Certified Nursing Assistant (CNA). Typically reports to a manager or head of a unit/department. The Resident Care Coordinator supervises a small group of para-professional staff in an organization characterized by highly transactional or repetitive processes. Contributes to the development of processes and procedures. To be a Resident Care Coordinator typically requires 3 years experience in the related area as an individual contributor. Resident Care Coordinator

  • Provides nursing care for residents according to established standards and practices relating to basic nursing, restorative and personal care services where quality assurance, resident safety, and infection control are inherent responsibilities. 
    • Adheres to the scope of practice for nursing licensure per state regulatory guidelines.
    • Maintains and monitors MARs and EMARs on a daily basis and ensures accuracy. 
    • Works to coordinate, support and supervise home healthcare and hospice services for residents.
    • Ensures all documentation is complete and current while maintaining the standards set by the state and company.  
    • Reviews and follows up on incident reports to ensure state reportable incidents are being reported correctly and proper procedure is being followed per state mandated guidelines. 
    • Processes and implements physician orders accurately and promptly using nursing judgement. 
    • Conducts all inventory of all medical/nursing supplies and maintains adequate supplies. 
    • Monitors careful and efficient use of supplies, checks that all medical equipment is clean, in working order and up to code per state guidelines. 
    • Maintains up to date physician orders, care plans and history and physicals per state guidelines. 
    • Prepares and participates in care conferences. 
    • In conjunction with the resident care coordinator, responsibilities include interviewing, hiring and training new employees, performance reviews, disciplining employees, addressing complaints and resolving problems. 
    • Verifies training and competency of certified medication aides where permitted. 
    • Provides accurate and timely information regarding resident assessments and status to management company for HHS data collection and submit monthly healthcare reports.
    Managerial Responsibilities: 
    Supervises the RCC as well as on duty caregivers and medication aides. 

    Qualifications:
    • Minimum of three years experience in assisted living and memory care communities. 
    • Minimum of three years supervisory or leadership experience.
    • Completion of a professional nursing program and licensed to practice as a registered or licensed nurse. 
    • Ability to work evenings, weekends, holidays and flexible hours to meet the needs of our residents. 
    • Must have high level of interpersonal skills to handle sensitive, confidential information and maintain the utmost professionalism in every situation. 
    • Must have excellent verbal, oral, written and communication skills.

    Job Summary

    The Resident Care coordinator works as a liaison between residents, residents' families, and staff. The Resident Care Coordinator's duties also include problem-solving resident concerns and coordinating care with the Caregivers. The RCC may be an RN or LPN when necessary.

    Minimum Eligibility Requirments

    • Ability to communicate effectively with Residents, families, staff, vendors, and the general public
    • Must have compassion for and desire to work with the elderly and understand that for each resident the facility is considered the Resident's home
    • Must demonstrate the ability to work responsibly as a team member as well as an individual
    • Must be honest, ethical, fair, dependable, respect confidentiality and the rights and privacy of others
    • Must meet all health requirements
    • Must pass a criminal background check
    • Must be able to perform duties and responsibilities (essential job functions), with or without reasonable accommodation

    Essential Functions

    • Caregiver scheduling and resident assignments, working within the department allowances
    • Coordinate admissions with marketing and Administrator to be sure accommodations are as delivered and care is immediately implemented
    • Family/Resident admission interviews
    • Immediately bring prohibited conditions or at-risk Residents in need of a higher level of care to the attention of the Administrator
    • Coordinate care planning within community policy
    • Arrange transportation as desired by the Resident
    • Arrange for resident special needs involving other departments, verifying follow through
    • Function as a liaison with families ensuring particular needs/requests/complaints are addressed
    • Inform the Executive Director, in writing and verbally, of all family or Resident complaints
    • Monitor staff performance, providing or arranging assistance as needed
    • read all communication notes regarding the community between the caregiver shifts
    • Coordinate staff training and in-service schedules with the Administrator
    • Supervise the medication room and orders, working with the supervising Medication Aides and Caregivers
    • Other duties assigned
    • Required to stay alert and attentive during scheduled work hours

    Customer Service

    • Responsible for ensuring that all employees are providing excellent customer service to internal and external customers
    • Perform job duties for Residents and team members in a courteous and professional manner
    • Taking the initiative to ensure resident safety and satisfaction is a priority
    • Meet and greet visitors in a friendly, helpful manner
    • Communicate professionally and cooperate with the supervisor and all community personnel

    • Ensure compliance and understanding of all regulations regarding residents' rights

    Working conditions

    • Scheduled hours; possible overtime

    • Is involved with residents, personnel, and visitors under all conditions and circumstances

    Physical/sensory requirements

    • Medium work: the ability to exert 10-35 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to lift, carry, push, pull, or otherwise move objects
    • Must be able to move intermittently throughout the day
    • Must function independently and have flexibility, personal integrity, and the ability to work effectively with Residents and staff
    • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times, hostile people within the community
