Morris Park Rehabilitation and Nursing Center is currently seeking a Resident Care Coordinator/Concierge with one year of long term care experience. This role is Monday thru Friday from 12pm to 8pm.  The ideal candidate needs to have good interpersonal skills, be self-motivated and have the ability to work without the need for constant oversight. The Resident Care Coordinator/Concierge needs to demonstrate good written and verbal communication skills.

This position supports the admissions department’s activities in the pre-admission, admission, discharge, and follows up processes. This position reports to the Director of  Human Resources.

Essential Functions:

• Ensures completion of Admission Agreements; files and tracks accordingly
• Notifies departments of anticipated admissions
• Checks rooms prior to admission and immediately notifies proper department and RN Nurse Manager of any issues/concerns
• Welcomes patient on admission.
• Assists with resident feedings when needed
• Meet with new admissions daily for a period of 7 days and re-admissions for 2 days to ensure all needs/expectations are being met and concerns addressed
• Meet with all residents throughout the week to ensure each resident is comfortable and happy with accommodations and care
• Complete daily report outlining resident questions, comments or concerns; note how concern was or will be addressed/rectified
• Inform residents of recreational activities and encourage participation
• Assist in transport of residents to dining room and recreation programs & events
• Update SIGMA daily
• Assist nursing staff with the following:
• arm bands
• room door and closet ID’s
• labeling of wheel chair and equipment
• Cover reception desk as needed
• Maintains a positive and professional demeanor toward residents, visitors, families, and co-workers.
• Adheres to all policies and procedures of the Nursing Home.
• Performs other duties as assigned.

The successful candidate will have the following qualifications/experience

·         High School Graduate or GED required

·         Strong written and verbal skills

·         Bilingual; Spanish and/or Italian strongly preferred

·         Proficient in Word, Excel and Outlook

·         Detail oriented

·         Ability to multitask and meet deadlines
