Resident Care Coordinator

The main function of the Resident Care Coordinator is to coordinate, lead and direct the care and services provided to residents by nursing assistants, med techs and other professionals or paraprofessionals. May also provide direct resident care as required.

Your Responsibilities

• Manage the healthcare of all residents within the community, including the dissemination of information to families and staff
• Assist the Health and Wellness Director in medication management, including administration of medications to residents and ordering medication from pharmacy
• Provide direct supervision of care given within the community
• Ensure all residents are treated with respect and dignity, recognizing individual needs and encouraging independence
• Would be responsible for scheduling all CNAs and willing to fill in on shifts whenever needed
• Would be expected to keep up with paperwork
• Responsible for helping oversee the med management with the lead med techs
• Assist the Director of Nursing or Executive Director for overseeing CNAs - hiring, disciplines, scheduling, etc
• Assist the Director of Nursing or Executive Director in the creation care plans, continuing to keep them up to date, and ensuring they match the nurse's assessments
• Primary focus should be assisting the residents in all areas of care and coordinating with the facility nurse, physicians, families, care staff, and the administrator to meet the needs of the residents
• Participate in the care planning process as assigned. 
• Assist nursing assistants with facility procedures for admission, discharge and transfer of residents.
• Consistently work cooperatively with residents, residents’ representatives, facility staff, physicians, consultant personnel and ancillary service providers

Qualifications

• Bachelor's degree or equivalent; minimum of 5 years of previous long-term care experience; or an equivalent combination of education and experience
• Prior experience managing staff and working in a long-term and/or assisted living setting preferred
• Experience with the applicable state Survey Rules and Regulations, Medicare, and Medicaid
