Required Qualifications (as evidenced by an attached resume):
Bachelor’s degree (foreign equivalent or higher) in Nursing or related fields. A valid New York State Registered Professional Nurse License. Two (2) years of full-time case management experience.
Preferred Qualifications:
Additional years (3 ) of full-time case management experience. Two (2) years full-time nursing experience in either a hospital, ambulatory or medical office setting. Experience with Electronic Medical Records systems. Experience with assisting patients with chronic illnesses, including cancer. Mental health experience, particularly in post-traumatic stress or depression. Experience with developing and implementing treatment plans or Plan of Care notes. Experience with Workers’ Compensation and working with 9/11 Responders. Proficiency with Microsoft Office.

The Stony Brook WTC Health Program offers comprehensive, integrative healthcare of 9/11-related illnesses for WTC disaster responders. Integral to our mission of patient-centered care is an enduring support community, advocacy for our 9/11 responders, and cutting-edge research efforts. The Medical Case Manager must possess the knowledge and experience to assess, plan and intervene as needed, to promote health, secure services and help patients cope with serious health issues. The incumbent must be able to work independently and as a member of a team. The successful incumbent must be detail oriented and have strong oral and written communication skills.
Duties:
- Responsible for care/case management, primarily for World Trade Center Health Program (WTCHP) patients with complex aero digestive health conditions, mental health conditions, and/or cancer.
- Assist Member Services and Case Management Administrator with the continuous development of systems within the program. This includes special projects and workgroups that serve to assist our population.
- Special projects include assisting member services and case management administrators create an Intensive Case Management support group and a Mental health compliance program.
- Assist with evaluating and measuring the quality of the case management team.
- Educate patients re: WTC Health Program benefits and limitations.
- Perform appropriate referral and disposition planning.
- Pre-authorize covered tests, procedures and treatments.
- Provide support services to patients and families.
- Assist in the clinic as needed for assessments and medical / mental health screenings in order for referring out members for services and treatment.
- Other duties as assigned.

The Research Foundation of SUNY is a private educational corporation. Employment is subject to the Research Foundation policies and procedures, sponsor guidelines and the availability of funding. FLSA Exempt position, not eligible for the overtime provisions of the FLSA. Minimum salary threshold must be met to maintain FLSA exemption.
**The incumbent must be willing to work and travel between the Nassau and Commack clinic locations. Occasional Evenings and Saturdays will be required.
