JOB DESCRIPTION
The Director of Home Care, in conjunction with discipline leaders is responsible for the supervision, planning, and coordination of patient care for patients and families to ensure quality delivery of hospice care. The Director of Home Care is responsible for ensuring around-the-clock nursing coverage and compliance with all state and federal regulations. The Director of Home Care works closely with the Executive Director of Clinical Services and the CEO to assure resources are made available to assigned counties. This position is an active member of the Directors Team and serves as clinical administrator on call. Work in the office and remote is approved and is coordinated with the supervisor to meet the needs of the organization.

The employee must be able to lift and/or move at least forty pounds using proper body mechanics and safety measures. Individual job responsibilities may require lifting more than 40 pounds on occasion.
