    Responsibilities:

    • Supervise a team of home care staff, including hiring, training, scheduling, and performance management.
    • Conduct regular performance evaluations and provide constructive feedback to improve staff performance and ensure adherence to organizational standards.
    • Develop and maintain comprehensive care plans for clients in collaboration with other healthcare professionals and family members.
    • Ensure compliance with all relevant regulations, policies, and procedures governing home care services.
    • Monitor client progress and make adjustments to care plans as necessary to meet evolving needs.
    • Coordinate with clients, family members, and healthcare providers to ensure continuity of care and address any concerns or issues that may arise.
    • Maintain accurate and up-to-date records of client assessments, care plans, and service delivery.
    • Provide ongoing support and training to home care staff to enhance their skills and knowledge in delivering quality care.
    • Conduct regular team meetings to communicate updates, address concerns, and facilitate collaboration among team members.

    Qualifications:

    • Bachelor's degree in health science, health behavioral science, health and human services, nursing, social work, healthcare administration, or a related field (preferred).
    • Previous experience in home care or a similar healthcare setting, with a minimum of 2-3 years of supervisory experience.
    • Strong leadership and communication skills, with the ability to effectively manage a team and communicate with clients, family members, and other healthcare professionals.
    • Knowledge of relevant regulations, policies, and procedures governing home care services.
    • Excellent organizational skills and attention to detail, with the ability to prioritize tasks and manage multiple responsibilities simultaneously.
    • Compassionate and empathetic attitude towards clients and their families.
    • Proficiency in basic computer applications, including electronic health record systems and Microsoft Office Suite.
    • Valid driver's license and reliable transportation (if required for home visits).
    • CPR and first aid certification (preferred).
    • Ability to pass background checks and other screening requirements as mandated by regulatory agencies or employer policies.

    Job Type: Full-time

    Pay: $54,000.00 - $64,000.00 per year
