Rehabilitation Services Supervisor - Home Care supervises the delivery of occupational therapy, physical therapy, and speech/language pathology programs to patients in a homecare setting. Responsible for providing consultation and monitoring of the performance of the rehabilitation staff. Being a Rehabilitation Services Supervisor - Home Care provides clinical consultation. Requires a bachelor's degree. Additionally, Rehabilitation Services Supervisor - Home Care typically reports to a manager or head of a unit/department. Requires a license in the area of specialization. The Rehabilitation Services Supervisor - Home Care supervises a group of primarily para-professional level staffs. May also be a level above a supervisor within high volume administrative/ production environments. Makes day-to-day decisions within or for a group/small department. Has some authority for personnel actions. To be a Rehabilitation Services Supervisor - Home Care typically requires 3-5 years experience in the related area as an individual contributor. Duties:
- Provide skilled nursing care to patients in their homes
- Conduct comprehensive assessments of patients' health status
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed
- Monitor and evaluate patient progress and adjust care plans as needed
- Educate patients and their families on disease management, medication administration, and self-care techniques
- Coordinate with other healthcare professionals to ensure continuity of care
- Perform Durable Medical Equipment (DME) setup and maintenance
- Collect and process specimens for laboratory testing
- Provide health coaching and support to patients and their familiesQualifications:
- Valid Registered Nurse (RN) license in the state of practice
- Minimum of 1 year of nursing experience, preferably in home health or a related field
- Proficiency in electronic medical record systems, such as Note-E-Fied, DataSoftLogic, etc.
- Strong assessment, communication, and documentation skills
