We are seeking an experienced Hospital Liaison to join our dynamic team. The ideal candidate will be a self-motivated and results-driven professional with established relationships with local and regional hospitals. The primary role of the Hospital Liaison is to generate new patient referrals and convert these referrals into admissions for our skilled nursing and rehabilitation center.
**Responsibilities:**
- Develop and maintain strong relationships with hospitals, social workers, case managers, and other referral sources to increase patient referrals.
- Conduct regular visits to hospitals and healthcare facilities to promote our services and establish a strong presence in the market.
- Understand and communicate the unique benefits and services of our facility to potential referral sources.
- Work closely with the admissions and clinical teams to ensure a smooth and efficient admission process for patients.
- Monitor and analyze market trends and competitor activities to identify opportunities for growth.
- Participate in community events and networking activities to enhance the visibility and reputation of our facility.
- Maintain accurate records of all activities and interactions with referral sources.
**Requirements:**
- Proven experience as a Hospital Liaison or similar role in the post-acute setting, with established relationships that can lead to referral generation.
- Strong understanding of the healthcare market, including key referral sources and competitors.
- Excellent communication and interpersonal skills, with the ability to build and maintain positive relationships.
- Demonstrated ability to work independently and as part of a team.
- Strong organizational and time management skills, with the ability to prioritize tasks effectively.
