Drug Treatment Navigation Case Management and Referral Specialist

  • The HRS Case Manager will work up to 40 hours per week. Regular hours are Monday-Friday 10am-6pm. This position will require the Case Manager to attend and pass various trainings, including the State Office of AIDS’ HIV test counselor certificate program (if not already certified, we will certify), participation in health related outreach and resource development. Position may include limited evening commitments.

    Full-time HRS employees that have worked for 90 days receive 18 paid holidays (10-12 is standard in other companies and organizations), 10 vacation days, 12 paid sick days, and currently enjoy fully paid health and dental insurance!

    Harm Reduction Services (HRS)

    HRS is a Harm Reduction-based non-profit organization that offers a non-judgmental, non-threatening delivery of public health-centered services and resources. Services include but are not limited to Medical Case Management, HIV/Hep C Testing, Syringe Exchange, and much more.     What exactly do you mean by “Harm Reduction?”

    Harm Reduction is grounded in justice and human rights – it focuses on positive change and on working with people without judgment, or discrimination. It is the belief that making practical, safer, and realistic steps toward reducing one’s harm will create long-term positive change in individuals. The primary goal of harm reduction is to reduce the negative effects that may be experienced with substance use while not challenging a person’s right to use substances. The secondary goal is to provide people with resources when they do decide they would like to stop using substances. The underlying principle behind harm reduction is that the use of drugs does not mean people forfeit their human rights.

    Case Manager Responsibilities

    · Provide Syringe Exchange and Disposal Services as well as Risk Reduction and Health Education to clients that visit our Harm Reduction Center.

    · Provide educational assistance and appropriate referrals to clients interested in alcohol and drug treatment, including Detox, MAT (Methadone and Suboxone) and residential treatment.

    · Provide case management assistance for individuals navigating drug treatment services.

    · Keep files in compliance by doing monthly updates with clients.

    · Keep case notes up to date daily.

    · Transport clients to and from drug treatment related appointments in your own vehicle.

    · Assist individuals with community resources such as health care and insurance, mental health series, food access and shelter.

    · Maintain confidentially and HIPPA protocol while in office and out in the field.

    · Provide Overdose Recognition and Response Trainings (naloxone) to community members and program participants.

    · As the first point of contact: Provide each individual that walks through our doors with compassion, dignity and respect. Conduct community outreach and clean-up around our site and periodically with our outreach team to locations around Sacramento County.

    · Perform data entry and collection on a regular basis and as requested/

    · Stock all supplies and educational materials.

    · Answer phones, relay messages and greet participants and guests.

    · Perform HIV/HCV/STI testing and counseling with participants.

    · Participate in trainings as requested.

    · Other duties as assigned.

    Basic Requirements:

    · Physical health that allows the employee to use public and private transportation, to walk distances up to 1 mile several times a day and work their assigned tasks for periods up to 8 hours per day with appropriate breaks. Duties may occasionally include lifting up to 40 pounds, climbing a flight of stairs and light cleaning of the office area.

    · This job absolutely requires use of employees own vehicle on a daily basis and therefore requires that employee has reliable vehicle, with appropriate automobile insurance, a valid California driver’s license, and a safe driving record. Mileage will be reimbursed at the federal rate which is currently $0.67 per mile. The worker may be assigned to operate vehicles owned by the agency as well.

    · Literacy that allows the worker to complete record keeping forms necessary to the data collection and reporting associated with this position. This will involve report writing and some grant requested tasks.

    · Experience with data collection, entry and analysis that will allow the individual to complete and maintain data collection instruments and systems.

    · Ability to develop relationships with clients and other providers to is key to the program’s success.

    · Effective communication and problem solving skills and experience dealing with diverse individuals and communities. Possess the skills necessary to display client-centered, non-judgmental services to all prospective clients, and to participate in planning and coordination meetings with others from the service community.
