    JOB SUMMARY : Duties :

    Duties :

    Performsquality functions in support of the ambulatory division.

    Reconciles and captures clinical quality documentation.Assists with health plan medical record review (MRR) surveys andcompiles data for quality measure / health plan reporting.

    Assists withmanaging the data and clinical quality integrity of the medicalrecord.

    Collaborateswith the Manager to enhance quality measure provider and stafftraining clinical performance and process improvement efforts

    Skills :

    Supports qualityoperations of the Ambulatory Division providers and clinical staff.

    Processes qualityand clinical results data for medical records pertaining to caregap closures preventive chronic conditions and continuity of care.

    Processes medicalrecords and supplemental data and corresponding results inaccordance with medical record policies for medical recordsreceived from outside facilities thirdparty vendors lab results andother external clinical data (i.

    e. updating health maintenancepreventive and chronic condition care gaps lab and procedureresults).

    Utilizesclinical knowledge to review provider and staff documentation andupdates medical record data received to ensure compliance withHEDIS and CMS measures through identified through health plan caregap lists.

    Utilizesclinical knowledge of vaccinations and immunization schedulesupdates the medical record immunization profiles and CaliforniaImmunization Registry (CAIR) to accurately reflect immunizationcompliance.

    Useclinical knowledge and judgment to assess external medical datareceived for appropriate inclusion in the medical record andsupplemental data to health plans.

    Assists managers in identifying addressingand resolving patient medical record care gaps and issues; providescorrective feedback to team members when appropriate.

    Utilizes clinical knowledge toperform medical record and immunization review / audits incompliance with CDPH CHDP and HEDIS reporting requests andrequirements.

    Assists in patient outreach and identifying andresolving care gap closure lists in the medical records.

    Performs dutiesrelated to policies associated with multiple missed appointmentsand disenrollment from the ambulatory departments.

    Complies with clients policiesand procedures HIPAA safety and other applicable regulatorystatutes.

    Participates in process management activities andimplements action plans.

    Performs all other duties as assigned.

    Education :

    AA degreeis preferred.

    Graduate of an accredited Medical Assistant programrequired.

    Must havesuccessfully completed the client Healthcare MedicationAdministration competency credential.

    Licensure / Certification : Certified MedicalAssistant required.

    Experience :

    Minimum 3 years of experience working in a medicaloffice in an outpatient setting.

    Skills :

    Excellent oral and written communicationskills and comprehensive knowledge of state and federal chartcompletion and release of information requirements.

    Knowledge of and experience withcomputers and computer systems.

    Ability to manage multiple tasks at one time.

    Demonstrated abilityto lead others in complying with policies and procedures.

    Working knowledge ofMS Word and MS Excel.

    Past experience with electronic medical records (i.e.Cerner applications or equivalent systems) required.
