Principal Functions:
Functions under general supervision and direction of the Admissions Manager , to plan, direct, and evaluate potential patients through a referral system, in accordance with established policy and process.
Assesses the needs of patients in accordance with the initial admission assessment program, and initiates, implements and evaluates individual patients for acute criteria for psychiatric hospitalization. Reviews information gathered and communicates effectively with the Admissions Manager and Medical Director or their designee for patient admission. Identifies candidates who meet the program criteria and who will benefit from program services.
Builds positive relationships with area skilled, independent, and assisted living facilities, physicians, physician office staff and hospitals. Markets Assurance Health and its services to assigned area hospitals. Helps to educate discharge planners, physicians and other appropriate referral sources for admissions into the facilities. May be requested to round with physicians, hospitalists, and others.
Position Duties:
- Identifies potential patients, performing on site level of care assessment for admission to Assurance Health. These settings can include, but are not limited to: acute care facilities/hospitals, and skilled/assisted/independent living facilities, residential or day care facilities, or private residences.
- The admission assessment and admission process is to be completed in a timely manner.
- Coordinates the patient admission, following established processes, which include, but not limited to: completion of all consent paperwork, ambulance transport scheduling, admission call/email notification, handbook distribution, and all other assigned admission process duties. Duties and process are subject to change with any/all process improvement implementation.
- Educates patients, family members or significant others, referral sources, hospital personnel, external payers, external case managers, and others as requested/required, as to the hospitals program and facilities.
- Demonstrates positive communication between customers, co-workers, and supervisors at all times.
- Maintains high ethical standards.
- Maintains a thorough knowledge of facilities, products, services, and acuity trends.
- Prepares and submits activity reports/logs at the direction of the Admissions Manager or Director of Patient Relations.
- Attends meetings as requested/required.
- Develops rapport with referral sources, clients, and their family members.
- Conducts other duties including exhibiting and providing tours.
- Promotes good public relations with patients, healthcare providers, and other involved people.
- Understanding that this position requires constant communication with the team, employee is available by phone at all times during on-the-clock hours. Promptly responds to calls/texts.
- Markets to assigned hospitals, including emergency departments, case management teams, and various other hospital departments. This includes cold-calling, conducting in-services, trainings, and various other educational methodologies to ensure all hospitals are familiar with the products and services of Assurance Health.
- Complies with federal and hospital requirements in the areas of protected health information and patient privacy.
- Performs discharge follow up analysis in accordance with company process and at the direction of the Admissions Manager and/or Director of Patient Relations.
- Maintains up to date log/report of discharge follow up activity and disposition.
- Relays pertinent feedback, positive or negative, to Admissions Manager for strategic resolution (if applicable).

Minimum Qualifications:
Education:
Must be a graduate from an accredited school of nursing, Social Work, or related field.
Experience: Prefer 6 months of RN, LPN, LMHC, Social Work experience. Prefer experience marketing and or community relations.
Licensure:
Currently licensed in the State of Ohio as a Registered Nurse, LPN, LMHC. CPR Certified.
Skills:
Must demonstrate expertise in analysis, decision making, time management, oral and written communication, and computer use. Familiarity with the resources available for use in assessing regulations. Good assessment skills and understanding of the insurance and managed care arenas. Ability to function independently. Able to communicate the hospital objectives and goals to outside individuals. Able to effectively seek out and communicate to outside referral sources.
Working Conditions:
The employee may be exposed to blood borne pathogens, infectious diseases, and other hazardous wastes, (i.e. Hepatitis, Aids, Tuberculosis). Proper precautions will be taught and provided by the hospital. The position requires a considerable amount of walking, stooping, pulling, lifting, feeling, hearing, and repetitive motions, occasionally overhead and low reaching, and to exert force of up to 50 lbs. occasionally, up to 10 lbs. frequently. May be asked to assist with greater weights.
